RUNTUHNYA GARDA TERDEPAN DI BUMI RAFFLESIA

MENGIKUTI perkembangan peningkatan kasus covid-19 di Indonesia, maka per tanggal 09 Mei 2020 menurut data yang dirilis oleh situs resmi covid19.go.id, maka jumlah kasus saat ini di Indonesia adalah 13645 Kasus dengan 2.607 sembuh dan 959 meninggal.

Sementara itu, untuk provinsi Bengkulu tercatat jumlah terkonfirmasi positif sebanyak 37 Kasus yang meliputi 8 (Delapan) Kabupaten/Kota minus Lebong dan Rejang Lebong (mudah-mudahan 2 kabupaten ini benar-benar datanya valid). Diberitakan melalui berbagai media, bahwa suspect dominannya adalah tenaga kesahatan.

Upaya dalam pencegahan penyebaran Covid-19 telah banyak dilakukan pemerintah pusat dan daerah. Beberapa Menteri terkaitpun sudah mengeluarkan kebijakan untuk para Aparatur Sipil Negera untuk mengurangi kontak fisik dan sosial dengan cara Work From Home (WFH). Begitu juga dengan sebagian besar Perguruan Tinggi Negeri (PTN) di Indonesia.

Namun berbeda halnya dengan tenaga medis dan perangkat pendukungnya, mereka di kecualikan dari kebijakan ini, sebab mereka adalah instrument terdepan sebagai garda penanganan wabah ini. Harus diakui bahwa para dokter dan pekerja medis yang berjibaku merawat dan berusaha menyembuhkan pasien penderita corona adalah pahlawan.

Terdapat 368.822 tenaga medis yang terjun langsung menangani wabah corona. Mereka ini terdiri dari 1.976 dokter spesialis paru, 6.656 dokter spesialis penyakit dalam, dan 30.678 dokter umum. Sedangkan, 329.512 orang lainnya adalah perawat.

Merekalah yang paling berisiko terpapar karena profesi yang digeluti, meski dokter lain pun tak tertutup kemungkinan juga dapat tertular dari pasien. Menyayat hati membayangkan dari jajaran angka-angka tersebut satu per satu tumbang oleh Sars-Cov-2 atau Covid-19. Meski sudah menjadi tugas dan kewajiban dalam keseharian, menangani pasien corona saat ini sama dengan terjun ke medan laga. Risiko ada di depan mata, yakni ikut terpapar dan bahkan meninggal.

Dua alternatif resiko tersebut sesungguhnya sudah terlihat sejak mewabahnya kasus ini. Khusus di Provinsi Bengkulu, hingga saat ini menurut data terakhir tercatat sejumlah tenaga kesehatan yang terpapar virus corona ini.

Menurut kepala Dinas Kesehatan Provinsi Bengkulu 6 orang dokter, dan 10 orang tenaga medis lainnya. Mereka ini merupakan tenaga medis yang langsung menangani kasus pasien COVID-19
Menurut analisis tracking yang telah dilakukan oleh tim gugus Covid-19 Provinsi Bengkulu, terpaparnya tenaga medis tersebut disebabkan oleh interaksi dengan pasien yang terkonfirmasi positiv Sars-Cov-2 atau Covid-19 dengan tanpa gejala, sementara diketahui tidak banyak informasi yang jujur diperoleh mereka dan juga tidak digunakannya (tidak tersedia) Alat Pelindung Diri yang sesuai standar ketika menangani pasien tersebut.

Belum lagi data yang diperoleh pada tanggal 27 April 2020 yang menyebutkan bahwa sudah banyak tenaga kesehatan yang meninggal dunia, diantaranya dari Ikatan Dokter Indonesia (IDI) menyebutkan sudah ada 24 Dokter, kemudian dari Ikatan Dokter Gigi Indonesia (IDGI) sebanyak 6 orang dan perawat 17 orang. Data ini bisa saja akan terus bertambah seiring dengan semakin meningkatnya jumlah tenaga kesehatan yang terpapar.

Peningkatan jumlah pasien yang terkonfirmasi positif ini menggambarkan bahwa tranmisi virus belum akan berhenti dalam waktu dekat, dan bisa saja akan membutuhkan waktu yang lebih lama dari perkiraan banyak pihak. Proses penanganan terhadap wabah ini disinyalir disebabkan oleh berbagai faktor, diantaranya; ketersediaan APD, keseriusan pemerintah untuk fokus penangananya dan kesadaran masyarakat.

Kekurangan alat pelindung diri (APD) merupakan persoalan yang paling banyak mengemuka dikalangan tenaga medis, disamping persoalan lainnya seperti kelelahan karena menangani pasien, beban psikologis para pekerja medis di Indonesia ditambah lagi oleh stigma negatif sebagian masyarakat dan lain sebagainya. Sejauh ini, bentuk apresiasi oleh pemerintah adalah dengan menganggarkan alokasi tambahan belanja APBN sebesar Rp 300 miliar untuk santunan kepada para dokter, perawat.

Hasil penelusuran penulis, anggaran dengan jumlah 15 Miliar dari APBD Provinsi (belum termasuk dari Kabupaten/Kota) untuk bidang kesehatan dalam penanganan kasus ini memang cukup besar, dan pertanyaannya adalah apakah anggaran tersebut sudah direalisasikan bagi pengadaan APD yang menjadi kebutuhan vital ini?, apakah sudah sesuai dengan standar yang terdiri handscoon,  tutup kepala, baju hazmat, sarung kaki, sepatu boots, masker N95, masker bedah, kacamata google, kemudian face shield dan sarung tangan panjang?
Hemat saya, berkenaan dengan APD ini, perlu kiranya perhatian pemerintah tidak hanya bagi tenaga medis yang langsung menangani pasien Sars-Cov-2 atau Covid-19 di Rumah Sakit Rujukan, melainkan juga diberikan minimal 1 set APD lengkap bagi tenaga medis mulai dari Klinik, Puskesmas, hingga Rumah Sakit non-Rujukan.

Saya bisa membayangkan dan tentunya akan sangat mengkhawatirkan apabila hal ini tidak diperhatikan secara serius, maka akan semakin banyak yang tumbang dan tentunya akan berdampak pada pelayanan kesehatan lainnya. Selain wabah Sars-Cov-2 atau Covid-19 yang sedang dihadapi saat ini, jangan lupa bahwa ada puluhan bahkan ratusan kegawatdaruratan lainnya yang harus ditangani mereka, mulai dari kecelakaan, melahirkan baik normal maupun caesar, jantung, cuci darah dan lain sebagainya.

Persoalan kedua yang dapat dilihat dari situasi ini adalah keseriusan pemerintah baik di tingkat mulai dari pusat, Provinsi, Kabupaten/Kota hingga ke lini terrendah dari sistem pemerintahan seperti RT. Strategi penanganan dan pencegahan dalam rangka mengatasi wabah Covid-19 ini tidak bisa dilakukan dengan pola ego sektoral, masing-masing tingkat haruslah memiliki garis koordinasi yang jelas dan terimplementasi dengan baik.

Sejauh ini yang terpublish di media adalah pencegahan dan perlindungan kepada masyarakat berkenaan dengan dampak covid-19 berupa pembagian beras, mie dan berbagai kebutuhan lainnya. Pola seperti ini juga disinyalir dimanfaatkan oleh beberapa pihak karena momentum politik rutin 5 tahunan. Hingga perdebatan yang muncul bukan soal efektifitas penanganan, akan tetapi ajang adu aksi baik dari aktor politik maupun dari partisan politik.

Pemerintah harus serius menjalankan protokol kesehatan yang telah ditetapkan, penegakan protokol kesehatan ini akan dapat membantu memutus mata rantai penyebaran Covid-19. Saat ini sudah tidak ada lagi toleransi lagi, pemerintah harus tegas menegakkan protokol kesehatan di Pasar-pasar, di Mall, di toko-toko, dan tempat keramaian lainnya. Bahkan jika memungkinkan dan sebelum terlambat, saya menyarankan agar segera mengusulkan Pembatasan Sosial Berskala Besar (PSBB) untuk Provinsi Bengkulu ini.

Persoalan ketiga yang tidak kalah pentingnya adalah kesadaran masyarakat akan wabah Covid-19 ini, baik dari resistensinya maupun pola penyebarannya. Sebagaimana diketahui, saat ini gejala Covid-19 tidak lagi sebatas batuk, flu, demam, tenggorokan gatal, melainkan sudah ada yang tanpa gejala. Ini artinya tidak ada yang bisa menjamin bahwa dalam dirinya tidak ada virus ini.

Oleh karena itulah maka masyarakat harus memiliki kesadaran akan upaya memutus mata rantai penyebarannya atau #janganngeyel alias #jangankereutak, salah satunya adalah tetap menjaga social distancing atau phsycal distancing (Jaga Jarak).

Implementasi jaga jarak tidak hanya menjaga jarak 1 atau dua meter dari lawan bicara, melainkan juga menjaga diri untuk tidak berinteraksi secara fisik dengan orang lain, itu artinya #dirumahaja. Hindari keramaian, jika tidak terlalu penting kiranya tetap #dirumahaja.

Kita tentu berharap pandemi ini segera berakhir, namun jangan terlalu memiliki ekspektasi yang tinggi agar cepat berakhir, jika masih mengabaikan tiga hal yang telah saya jelaskan di atas, Tenaga Medis, Keseriusan Pemerintah dan Kesadaran Masyarakat. Mudah-mudahan bermanfaat.
